Its key mechanism is anti-plasmin activity, helping suppress UV- and inflammation-triggered melanocyte activation by reducing plasmin-related mediator signaling, including pathways linked to prostaglandins, cytokines, and excessive melanin production.

In a randomized double-blind clinical trial, 5% topical tranexamic acid used twice daily for 12 weeks significantly reduced MASI score and melanin level in melasma patients, with fewer irritation-related side effects than hydroquinone 2%.
